New Insights into Challenges of Decentralized Clinical Trials for Patients and Researchers

A groundbreaking study reveals the benefits and challenges of decentralized clinical trials, highlighting impacts on research staff and patient experience. Balancing technology with human support is key to trial success.
A recent study conducted by researchers at University College Cork (UCC) sheds light on the complexities faced by clinical research staff and patients involved in decentralized clinical trials (DCTs). While DCTs leverage digital technology to make participation more accessible, they also introduce new hurdles that can impact trial outcomes and participant experiences. The study, published in the Journal of Medical Internet Research, involved interviews with clinical staff from several countries, including the U.S., Canada, Ireland, and Belgium.
The findings indicate that although remote, tech-enabled trial participation can reduce logistical burdens and improve accessibility, it may also weaken the personal connection between researchers and patients. Staff reported that building trust and understanding patient conditions become more challenging in a remote setting. Additionally, the shift to digital tools has led to increased workloads for research personnel, who now often handle troubleshooting, patient training, and platform navigation, highlighting the need for further support and training.
Patients, especially older adults, often struggle with unfamiliar technology, which can cause stress, confusion, and even withdrawal from the trial. This challenges the goal of patient-centric research and can negatively affect engagement, compliance, and retention. The study emphasizes that while technological advancements facilitate remote participation, they also require a thoughtful integration of human-centered design, behavioral science, and enhanced training programs.
Eoin Gamble, the lead researcher, stresses that the evolution of trial technology should aim to support both research staff and patients better. The goal is to create more inclusive, supportive, and effective trial environments as the landscape of clinical research continues to shift towards decentralization.
This research underscores the importance of balancing technological innovation with personalized human support to optimize the benefits of decentralized clinical trials for everyone involved.
