Artificial intelligence chatbots, powered by large language models (LLMs), have become increasingly accessible tools for answering questions and providing information around the clock. Their convenience and the breadth of data they draw upon make them appealing for many users. However, when it comes to complex and sensitive areas such as mental health and psychiatric medication reactions, their effectiveness is still limited.

Recent research from the Georgia Institute of Technology has highlighted these limitations by evaluating how well AI chatbots can identify adverse drug reactions (ADRs) associated with psychiatric medications and whether their advice aligns with that of human experts. The study, led by Associate Professor Munmun De Choudhury and Ph.D. student Mohit Chandra, introduces a new framework to assess AI responses in mental health scenarios, especially given the widespread lack of access to mental health care worldwide.

The researchers aimed to answer two key questions: First, can AI chatbots accurately detect when a patient is experiencing side effects or adverse reactions to psychiatric drugs? Second, if they can, do their recommendations and strategies for mitigation match clinical expertise?

To explore this, the team collected data from Reddit, a popular online forum where many individuals discuss medication experiences and side effects. They evaluated responses from nine different LLMs, including general-purpose models like GPT-4 and tailored medical models, comparing the AI outputs to clinically established answers provided by psychiatrists and psychiatry students.

The findings revealed that while AI models can mimic the tone and emotional expression of human psychiatrists—being polite and helpful—they often struggle with understanding the nuanced details of adverse drug reactions. Furthermore, although some responses may suggest harm-reduction strategies, they frequently lack the specificity and actionability that would make them genuinely useful and safe for patients. This discrepancy emphasizes the gap that still exists between AI-generated advice and professional medical guidance.

The implications of this research are particularly significant given the ongoing global shortage of mental health professionals and the increasing reliance of people on AI for health-related inquiries. Improving AI's ability to accurately detect side effects and provide reliable, actionable recommendations could potentially transform mental health support, especially in underserved communities.

Chandra underscores the importance of ongoing improvements in AI technology, advocating for policies and development efforts aimed at enhancing safety and effectiveness. As AI tools become more sophisticated, they could serve as vital resources, providing accessible, timely advice and support, but only if their recommendations are aligned with clinical standards.

This study highlights the ongoing journey towards creating AI systems that are truly reliable and safe for mental health applications, ensuring that these technologies support, rather than compromise, patient well-being.
