Transforming Workplace Culture Yields Significant Benefits in Healthcare Settings

A strategic approach to enhancing workplace culture at a California hospital has led to improved staff retention, patient outcomes, and organizational efficiency, setting a model for healthcare improvement.
A comprehensive initiative aimed at enhancing workplace culture at a major California hospital has resulted in substantial improvements in staff retention, patient outcomes, and organizational efficiency, as detailed in an article published in Critical Care Nurse. The hospital, managed by the Los Angeles County Department of Health Services and affiliated with UCLA, responded to the national nursing shortage intensified by the COVID-19 pandemic by implementing a strategic plan focused on fostering a healthier work environment.
Led by Chief Nursing Officer Nancy Blake, Ph.D., RN, CCRN, NEA-BC, the project was rooted in the AACN's Healthy Work Environment (HWE) Standards, first introduced in 2005 and updated in 2016. These standards emphasize key elements such as skilled communication, true collaboration, effective decision-making, appropriate staffing, meaningful recognition, and authentic leadership. The hospital’s multifaceted approach included establishing shared governance, creating a transition-to-practice residency for new nurses, engaging in leadership development programs like the AACN Clinical Scene Investigator Academy, and leveraging technology to improve communication and staffing systems.
Results from the initiative include a reduction in nurse turnover from 20.62% in 2022 to 10.44% in 2024, along with an increase in nurses obtaining specialty certifications and higher levels of education. The residency program alone successfully onboarded over 500 new graduates, achieving a 93% retention rate within the first two years—a significant rise from previous rates near 50%. Patient safety indicators also saw improvements, with marked decreases in urinary tract infections, bloodstream infections, falls, and pressure injuries.
The hospital's efforts have been recognized with multiple awards, including receiving over ten units of the AACN Beacon Award for Excellence and a third-place Bronze Eagle Award from the LA County Productivity and Quality Commission. The initiatives resulted in more efficient staffing practices, reduced reliance on staffing agencies, and improved hiring processes, contributing to overall healthcare quality and staff satisfaction.
