A comprehensive systematic review published in the journal Addiction examines the effectiveness of various interventions designed to alter public perceptions of vaping harm. The review analyzes 85 studies focusing on how communication strategies impact understanding of vaping risks among different age groups and how these perceptions influence vaping and smoking behaviors.

The study found that messages emphasizing the addictive nature and potential harms of vaping were generally successful in shifting perceptions, with a significant number of studies in both young people and adults showing positive changes. However, some messaging also led to the misconception that vaping is as dangerous as smoking, which can discourage smokers from switching to less harmful alternatives.

Importantly, the review highlights that perceptions of vaping harm strongly predict behavior. Warning that vaping is harmful can reduce its use among youth, but when adults mistakenly believe vaping carries the same or greater risks than smoking, they may be less inclined to quit smoking and switch to vapes. This misconception is particularly concerning because in England, over 70% of adults who smoke inaccurately believe vaping to be as or more harmful than smoking.

The findings suggest that clear, evidence-based communication is crucial. Interventions that accurately inform the public about vaping's relative risks tend to improve understanding without reinforcing false beliefs. Communicating that vaping, while not risk-free, is considerably less harmful than smoking, appears to be an effective approach.

The review also emphasizes the urgent need for public health campaigns to address misconceptions. Correcting false beliefs about vaping can promote its use as a harm reduction tool and support smoking cessation efforts. For instance, understanding that misperceptions hinder smoking cessation is vital; misinformation may prevent smokers from using vapes, despite evidence indicating they are more effective than nicotine replacement therapies.

As policymakers in England prepare to implement stricter regulations on vaping, ensuring the public has access to accurate information is increasingly important. The review advocates for targeted messaging that informs rather than misleads, to prevent unintended consequences such as smokers turning away from vaping and continuing to smoke harmful cigarettes.

Overall, this review underscores that perceptions about vaping are malleable and that strategic communication can significantly influence behaviors related to smoking and vaping, ultimately aiding harm reduction efforts.
