The Montreal Model of ketamine therapy combines psychotherapy and supportive environments, leading to more sustained relief from severe depression and treatment resistance.

Recent clinical research led by McGill University introduces the 'Montreal Model,' a novel approach to ketamine therapy for severe depression. The study demonstrates that combining ketamine infusions with psychotherapy and a therapeutic environment significantly extends the duration of antidepressant effects. Participants with treatment-resistant depression, many experiencing suicidal thoughts, showed an average depression score reduction of approximately 30%, with improvements in suicidality and anxiety. Notably, these effects persisted for at least eight weeks, unlike conventional ketamine treatments where benefits often diminish within days.

The study emphasizes the importance of the treatment environment—rooms designed with calming lighting, plants, curated music or guided mindfulness—alongside regular psychotherapy sessions. This approach underscores the significance of psychological context in treatment outcomes.

Developed during residencies at McGill by Dr. Greenway and Dr. Garel, the Montreal Model integrates traditional psychiatry with strategies inspired by psychedelic therapy, including creating safe, supportive spaces for patients. The model has been successfully applied in both public and private healthcare settings in Canada and internationally.

An intriguing aspect of the research is the link between the subjective mystical or emotional experiences during ketamine sessions and positive therapeutic outcomes. These findings challenge the traditional view of such experiences as mere side effects, suggesting they may be integral to the healing process.

The study's positive results support the adoption of structured, environment-centric ketamine treatments, offering a promising path for managing complex, treatment-resistant depression. It also highlights the need for standardized protocols as ketamine clinics proliferate across Canada and beyond.
